Physicals and Immunization Records

The Iowa Department of Human Services requires that all children enrolled in a child care center or preschool have on file a copy of a physical and immunization record.

Physicals must be updated at least annually. Click here to download a copy of our physical form to take with you to your child’s next appointment.

Immunization Records must be updated each time your child receives an immunization. Your child’s physician may fax a copy of your child’s record to directly to LilyPad at 515-597-5438.
